Donate by Mail - Joan's Run for the Leukemia & Lymphoma Society

You can send your donation directly to me and I will pass it along to LSS.

  1. Write out the following on a piece of paper, or print out and use the Donation Form:
    • The amount of your tax deductible donation (must be in US dollars)
    • Your name
    • Your full mailing address
    • Your phone number with area code

  2. You may pay by check, money order or credit card.
    • Make your check or money order payable to "The Leukemia & Lymphoma Society"
    • For credit card donations only, give:
      --- card type (VISA, MasterCard, or American Express)
      --- expiration date
      --- card number
      --- cardholder name (as shown on card)
      --- signature (sign your name)

  3. Your company may be willing to match your donation. Check with your Human Relations Department for the matching gift donation form, fill it out and mail it to me with your donation. I will send it on to the Leukemia & Lymphoma Society for separate processing. If you wish, you may send the matching donation form at a later time. Corporate Sponsorships are also available. Write me for more information.

  4. If you know someone who has or had leukemia or lymphoma, please let me know and I will run in their honor as well. If you say it's OK, I'll also add your person to my Honored Teammate Page with whatever information you provide.

  5. Mail the completed form, information and payment to:
